Referral Guide

For those filling out our referral form for the first time, we wanted to provide a bit of guidance to avoid any confusion. Here are the main points:

  1. Client's details

    This is where you put in the name and contact details of the person who will *receive* support. We can only help families living in Birmingham City Council postcodes. To check, please use Birmingham City Council's Find My Councillor form: https://www.birmingham.gov.uk/councillors/search

  2. Self-referrals

    We can only accept a self-referral from the family for unborn children. However, we may still need the request to come from support worker (see below). If there is no support worker but one is needed, this will flag up on our system and we will not be able to process your form.

  3. Support workers

    A support worker is someone who can fill in the form on a client's behalf. All support workers must give their full contact details, as they would do for the client, or there may be a delay with us processing your form. A support worker could include: midwives, social workers, family support workers, health visitors, and agency staff from charities and other organisations - who visit the family directly.   4. Reason for support

    Please choose one of the reasons for making the referral. If your reason isn’t listed please choose other and fill in more details in the box underneath.

  5. Details of children and parents

    Please double-check that you have the right age and gender of all children living at the property. This should include both the children in need of our support and any other children who live there. We also ask that you list the age and ethnicity of the parents.

  6. Items needed

    Please just tick the boxes for the items needed. We will help with everything asked for, but please do not just tick every box. This will lead to some items being wasted, and if these mistakes are made it will create issues down the line. If there is anything not covered, you can write it in the "Other" box. We cannot help with food parcels for adults, white goods, furniture or car seats.

  7. Sizes and dietary requirements

We need to know the exact sizes of nappies and clothes. We need the correct number and brand for milk. We also need to know here if the baby is vegetarian, lactose intolerant, or has any other dietary needs. We cannot guarantee brands on wipes, nappies or toiletries.
